Children and Family Treatment Support Services

Starting in 2019, Children and Family Treatment and Support Services (CTFSS) are available to all Medicaid-eligible children under the age of 21 who meet medical necessity criteria. CFTSS are new mental health and substance use services that give children/youth and their families the power to improve their health, well-being, and quality of life.

As you well know, early detection of cancer is key to positive patient outcomes. Screen your patients according to American Cancer Society guidelines.

Managing a condition can be overwhelming. We are here to help you by offering eligible members access to education and support through our Care Management Programs. These programs complement your doctor’s care and help you better understand and manage your condition to meet your health goals.

As you may know, diabetes is an epidemic In New York City. More than half a million adults have it and another 200,000 don’t know they have it. Help us reduce the number of diabetes cases by testing your patients for it, prescribing a diet and exercise program and providing proper treatment.

If you suspect that a patient of yours is the victim of domestic abuse, let them know they are not alone and that help is available.

EmblemHealth Neighborhood Care — a unique approach to solving the challenges of health care — is open to ALL members of the community. Invite your patients to take a tour of one of our locations and then discuss their health care issues with our nurses, pharmacists and care navigators. They can also discuss referrals, learn more about preventive care and find out about treatment options for existing conditions. We can also help with ways to lower out-of-pocket costs, resolve billing issues, assist with joining health and wellness programs, and connect you to community support.

The CDC’s Injury Center created the Stopping Elderly Accidents, Death and Injury (STEADI) Toolkit, a free resource guide to assist health care providers with the tools they need to assess/address their patients’ fall risk. This guide also helps with fall prevention conversations with your patients. The STEADI Toolkit can be downloaded or ordered online.
