Follow the steps below to complete your PDF online:
- Download and install Adobe Acrobat Reader
- Go back to the forms page and download the PDF you need to fill-out
- Open the downloaded PDF in Adobe Acrobat Reader
- Complete the form
- Click on File > Save As PDF
- Send applications to Credentialingnyc@emblemhealth.com.
