Partnership unites EmblemHealth’s long history of providing health care services to New Yorkers with Cityblock Health’s innovative community-based, technology-enabled services to address social factors that influence health.
New York, New York (April 18, 2018)– EmblemHealth, one of the nation’s largest non-profit health insurers, and Cityblock Health, a new company that uses technology-enabled services to address the unmet health and social needs of urban populations, have forged a unique partnership to bring a new model of care to New York. This comprehensive and integrated approach addresses social determinants and behavioral health as fundamental pillars of delivering services to those who are traditionally underserved.
At the leading-edge of a changing health care delivery system, the new high-touch personalized health system fully integrates primary care, behavioral health and social services, with custom-built technology to enhance strong relationships between members with complex needs and their care teams.

Recognizing the need for a place where health and community merge, EmblemHealth and Cityblock Health will open their first joint Hub at EmblemHealth’s Crown Heights location in the heart of Brooklyn. The Hub will provide a place for engagement between EmblemHealth members, Cityblock caregivers, and community-based organizations to address complex health needs and social factors at the neighborhood level. The partnership will also leverage Community Health Partners and an integrated, personalized care team who are able to assist members with all aspects of their health. Members will be able to access their care teams where and when they are most comfortable – in the Hub, in their homes, or in the community.
The foundation enabling this level of coordination is Cityblock Health’s novel care facilitation platform, Commons. The custom-built digital platform allows for seamless collaboration among care team members, including physicians, nurses, behavioral health clinicians, and Community Health Partners, to connect members with the right type of services and supports – clinical or otherwise – at the right time.
